Key Responsibilities
  • Deliver high-quality, person-centred care in clients’ homes and in our SIL
  • Support clients with daily living activities including personal care, mobility, and meal preparation
  • Provide compassionate care for individuals living with dementia and complex needs
  • Promote independence, dignity, and wellbeing at all times
  • Observe, document, and report changes in client condition to the nursing team
  • Follow care plans and work collaboratively within a nurse-led model
  • Build trusting relationships with clients and their families
  • Maintain professional standards, confidentiality, and compliance with policies and procedures
Desired Qualifications & Skills
  • Certificate III or IV in Aged Care, Disability, or Individual Support (or currently studying nursing)
  • Current First Aid and CPR certification
  • Manual Handling training completed within the last 12 months (or willingness to complete training provided by us)
  • NDIS Quality, Safety and You module
  • NDIS New Worker Orientation modules
  • Supporting Effective Communication module
  • Supporting Safe and Enjoyable Meals module
  • Current Police Check
  • Current WWVP card
  • Current Worker Screening ID
  • Current Infection Control training (or willingness to complete training provided by us)
  • Strong understanding of person-centred care principles
  • Experience in home care and/or dementia support (preferred)
  • Excellent communication and interpersonal skills
  • Ability to think critically and respond to changing client needs
  • Reliable, organised, and able to work independently
  • Commitment to high standards of care and professionalism
  • Current driver’s licence and willingness to travel between clients
