Key Responsibilities
- Deliver high-quality, person-centred care in clients’ homes and in our SIL
- Support clients with daily living activities including personal care, mobility, and meal preparation
- Provide compassionate care for individuals living with dementia and complex needs
- Promote independence, dignity, and wellbeing at all times
- Observe, document, and report changes in client condition to the nursing team
- Follow care plans and work collaboratively within a nurse-led model
- Build trusting relationships with clients and their families
- Maintain professional standards, confidentiality, and compliance with policies and procedures
Desired Qualifications & Skills
- Certificate III or IV in Aged Care, Disability, or Individual Support (or currently studying nursing)
- Strong understanding of person-centred care principles
- Experience in home care and/or dementia support (preferred)
- Excellent communication and interpersonal skills
- Ability to think critically and respond to changing client needs
- Reliable, organised, and able to work independently
- Commitment to high standards of care and professionalism
- Current driver’s licence and willingness to travel between clients
